Access Plan

$15 /month

  • Gives access to telehealth services 24 hours a day/7 days a week

Primary Care Plus Plan

$120/month

Children 2-18 $60/month

Gives access to telehealth and home visits 24 hours a day/7 days a week. All primary care services to include physicals, age recommended preventative health screenings, annual labs, follow-up, care personalized to you and your health needs. Telehealth visits are included in this plan at no additional charge. Home visits, as needed, will be $100/visit.

Primary Care Premium Plan

$250 /month

Children 2-18 $125/month

Covers all of your primary care needs, including annual physical and labs, repeated as necessary, in addition to 24 hour/7day a week access to telehealth and home visits at no additional charge, and care personalized to meet your health care needs.

Men’s Health Plan

$180 per month

Covers all primary care needs, including annual physical and quarterly labs with testosterone therapy for those requiring it. Telehealth is included in this plan. Home visits for quarterly labs are also included. Additional home visits will be charged at $100/visit.

All memberships provide access to care 24 hours a day/7 days a week. There will be an after hours charge  of $50 added to any Telehealth and $100 added to any home visits that take place between the night time hours of 10pm and 7am.

  1. Injectables: $10/unit; $8/unit with a party of 4 or more

Physical exam, lab draws,  simple laceration repair, abscess drainage, ear wax removal, strep swabs, respiratory swabs (flu, rsv, Covid), urine dip sticks, IV fluid hydration, migraine treatment, breathing treatments, appropriate medication administration as needed.

Detailed Services Information

Annual Physicals

Annual physical exams completed in the comfort of your own home. They include a head to toe assessment, age appropriate screening, diagnoses as deemed necessary, and a detailed care plan designed to meet your needs.

Sports Physicals

No membership fee is required for a complete sports physical at your convenience in the comfort of your home.  You can also book a time for your entire team and we will come to your practice or team meeting to complete physicals required for sport participation at a discounted rate.

Lab Draws

Our provider will come to you and draw annual labs and any other lab necessary to complete your care plan. No delayed appointments and no unnecessary wait as these will be completed at the time of your physical exam and again, in the comfort of your own home. Results are typically available within 48 hours at which time we will schedule follow up to review results and formulate a care plan that best suits your needs. Repeat labs can be collected as needed based on your specific care needs. 

IV Hydration

Feeling under the weather? Over extended yourself?  We can deliver IV hydration; administered at your home when you need it. Additional medications may be recommended and administered by your provider if deemed necessary to improve the clinical outcome.

Rapid Swabs

We provide rapid swabs included in the home visit fee or as part of your membership for COVID, Flu A/B, RSV, and Strep. Results provided with appropriate documentation at the time of your visit.
